A plan’s status is only part of the story.

This file does not show counties, benefits, or your personal eligibility. A plan can continue while leaving your area. Confirm your plan’s notice and 2027 coverage before making a decision.

Find your plan’s next chapter

2026 → 2027

Use the plan ID on your insurance card or plan documents. Enter plan information only—no Medicare number or personal details.

Hyphens and spaces are optional. A trailing segment (such as -000) is ignored; CMS does not supply segment-level details here.

Counts are workbook records matching your search, special-needs, and name-change filters—not members or unique plans. Status buttons exclude any exact-status filter.

Understand the result

CMS statuses, in plain language

The lookup keeps the original CMS status on every result. These labels help you interpret the source; they do not determine an individual member’s enrollment.

Continuing

CMS: Renewal Plan

CMS lists a renewal. Review the 2027 plan documents: premiums, benefits, doctors, and covered drugs can still change.

Continuing · area expanding

CMS: Renewal Plan with SAE

SAE means service-area expansion. The plan is renewing and adding service area. Check the plan’s 2027 documents and local availability.

Service area shrinking

CMS: Renewal Plan with SAR

SAR means service-area reduction. The plan renews in only part of its prior service area. This file does not identify affected counties. Check your plan’s notice before assuming you can stay enrolled.

Consolidated / crosswalked

CMS: Consolidated Renewal Plan

CMS lists a consolidated renewal into the 2027 plan shown. Confirm the destination and your eligibility with the plan, especially when more than one mapping appears.

Terminating / non-renewing

CMS: Terminated/Non-renewed Contract

CMS marks this record as terminated/non-renewed and lists no 2027 destination. Review your non-renewal notice and contact the plan, Medicare, or a licensed agent about your next steps.

New for 2027

CMS: New Plan

CMS lists a new plan with no 2026 predecessor in this row. This is not a destination for an existing plan unless another row explicitly maps to it.

New contract for 2027

CMS: Initial Contract

CMS lists an initial contract with no 2026 predecessor in this row. Availability and eligibility must be checked separately.

Before you decide

Questions about your plan change

Does a continuing plan mean my benefits stay the same? +

No. A renewal describes the plan’s year-to-year status. Premiums, benefits, provider networks, covered drugs, pharmacies, and costs can change. Review the plan’s 2027 Annual Notice of Change and coverage documents.

Why can one 2026 plan show several 2027 destinations? +

The CMS workbook can contain multiple mappings for the same 2026 contract and plan ID. It does not provide the county, segment, or member eligibility details needed to choose a destination for a particular person. Check your plan’s notice or ask the plan to confirm.

Can I search by ZIP code or compare benefits here? +

This source has no ZIP codes, counties, premiums, benefits, doctor networks, or drug formularies. Use Medicare.gov Plan Compare and the plan’s official documents to verify local availability and coverage. This tool does not cover Medicare Supplement (Medigap) policies.

What should I do if my plan is terminating or reducing its service area? +

Read the notice from your plan and confirm whether your coverage is affected. Review available options and enrollment timing with the plan, Medicare, or a licensed agent. A record in this tool alone does not establish your enrollment rights or enroll you in another plan.

How current is this lookup? +

It uses the CMS workbook PlanCrosswalk2027_10012026.xlsx dated October 1, 2026. All 9,144 source records and all seven original CMS status categories are preserved. This is a dated snapshot, not a live feed; later CMS or plan notices may supersede it.
